AMATEUR ATHLETIC WAIVER AND RELEASE OF LIABILITY READ BEFORE SIGNING In consideration of being allowed to participate in any way in Lone Pine Sports, LLC athletic sports program, related events and activities, the undersigned acknowledges, appreciates, and agrees that: 1. I am at least 18 years of age. 2. The risks of injury and illness (ex: communicable diseases such as MRSA, influenza, and COVID-19) from the activities involved in this program are significant, including the potential for permanent paralysis and death, and while particular rules, equipment, and personal discipline may reduce these risks, the risks of serious injury and illness do exist; and, 3. I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASEES or others, and assume full responsibility for my participation; and, 4. I willingly agree to comply with the stated and customary terms and conditions for participation. If, however, I observe any unusual significant hazard during my presence or participation, I will remove myself from participation and bring such to the attention of the nearest official immediately; and, 5. I, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, HEREBY RELEASE AND HOLD HARMLESS Lone Pine Sports, LLC their officers, officials, agents, and/or employees, other participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners and lessors of premises used to conduct the event (“RELEASEES”), WITH RESPECT TO ANY AND ALL INJURY, ILLNESS, DISABILITY, DEATH, or loss or damage to person or property, WHETHER ARISING FROM THE NEGLIGENCE OF THE RELEASEES OR OTHERWISE, to the fullest extent permitted by law. 6. Photo & Video Release: I hereby grant Lone Pine Sports, and its photographers/videographers, the right to use my likeness in any publication, both digitally and printed, without payment or other consideration. I understand and agree that any photographers/videos taken by Lone Pine Sports are the copyrighted work of their photographers/videographers and may be altered, edited, copied, printed, published or distributed at their discretion or the discretion of Lone Pine Sports. LONE PINE SPORTS Code of Conduct “Sportsmanship Contract” 1. Participation in Lone Pine Sports leagues emphasize fun and community. 2. Every Lone Pine Sports league encourages sportsmanship and safety. 3. Lone Pine Sports leagues do not tolerate profanity, rowdiness, poor sportsmanship or dangerous play. Such behavior can warrant ejection from a game or match. 4. The Lone Pine Sports director and officials, or their liaison, have the ultimate authority to eject a player from a game. 5. Any player, or non-playing spectator, who is ejected from a game will be suspended for at least one more game. 6. Any player or non-playing spectator, who is ejected from a Lone Pine Sports a second time is subject to suspension from Lone Pine Sports indefinitely. Any suspension is at the sole discretion of LPS. 7. A player or non-playing captain who has been ejected from a game can be asked to leave the premises by any office or Lone Pine Sports representative, if the behavior continues to be a distraction. Failure to leave could result in forfeiture of the game for their team. Please be advised that spectators whom were invited by players and captains should adhere to ‘good behavior’ or they are subject to ejection from the premises as well. I HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND SIGN IT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT.
